SHDHS Locker Lock Purchase Form

Every student will receive a locker for the 2024-2025 school year. This form is a request to have a “lock” for your locker. There is a $5.00 cost for renting the lock for the school year. Students who utilize the locks must use a school-sanctioned lock and keep it locked at all times. If not appropriately utilized, the administration can take it away.
